Does Ayurveda Treatment for Back Pain Have Scientific Evidence?

Yes. Evidence has shown the efficacy of different Ayurvedic methods of treatment for backache and other musculoskeletal disorders. Current evidence indicates that some forms of treatment can provide relief from pain and stiffness to the patients when included in general conservative non-surgical treatment protocols.   

At the same time, the results may vary according to the diagnosis, the degree of severity of the condition as well as the characteristics of a particular patient. That is why the application of Ayurveda should be personalized and not generalized.

Research Evidence

1. Ayurvedic Massage for Chronic Low Back Pain – Randomized Controlled Trial, 2017

A randomized trial involving 64 patients has shown that the use of Ayurvedic massage and Sahacharadi Taila together with local heating resulted in more effective decreasing of pain. According to the study results, the treatment was safe and properly tolerated.

2. Yoga + Kati Basti for Chronic Low Back Pain – Randomized Controlled Trial, 2024

In another randomized trial, the effectiveness of the integrative program including yoga and Kati Basti was evaluated against the program including yoga only involving 40 patients. The outcomes of the both groups indicated significant improvement in terms of pain perception and disability at one week and three months after the treatment.

3. Agnikarma vs Kati Basti in Sciatica – Comparative Clinical Study

A clinical trial of 40 people who had Gridhrasi (sciatica) along with lumbar disc prolapse compared Agnikarma and Kati Basti. All patients experienced good improvement in terms of pain relief and straight leg raising with Agnikarma being more effective compared to Kati Basti.

4. Nirgundi + Matra Basti for Sciatica – Clinical Study

A clinical trial of 119 patients was undertaken to assess Nirgundi Ghana Vati (herbal formulation), along with Matra Basti (medicated oil enema) to evaluate Gridhrasi. The trial assessed pain, stiffness, and other clinical symptoms of sciatica.

5. Ayurvedic External Therapies for Sciatica – Randomized Trial

A prospective randomized active control study of 60 patients evaluated external Snehan (oil application) with Bhujangasana and Shalabhasana. The results showed significant improvement in sciatica symptoms after the intervention.

6. Agnikarma and Raktamokshana for Lumbar Spondylosis – Clinical Study

A clinical trial of 32 patients with lumbar spondylosis indicated significant alleviation of lower-back pain, stiffness, loss of sensation, and difficulty with movement after administering Ayurvedic techniques such as Agnikarma (therapeutic heat) and Raktamokshana (therapeutic blood-letting).

What Is Back Pain According to Ayurveda?

According to Ayurveda, pain and stiffness in the back are usually due to the derangement of the Vata dosha (which consists of the air and space elements). This is particularly true when there is dryness, excessive movement, tissues that are too depleted, or other detrimental lifestyle factors.

Ayurveda considers the following causative reasons for back pain:

  • Abnormal increase of Vata dosha (Vata Prakopa)
  • Tissue depletion/degeneration (Dhatu Kshaya)
  • Impaired digestion by Ayurvedic principles (Ama)
  • Wrong posture and excessive force.
  • A sedentary lifestyle and prolonged sitting.
  • An irregular diet and routines.
  • Age using the system of the musculoskeletal system.

Ayurveda does not concentrate only on the pain area, but evaluates the person in totality. Such assessment is done through the following: Body Constitution (Prakriti), Current Imbalance (Vikriti), Digestive/metabolic capacity (Agni), lifestyle, and the nature of pain and accompanying symptoms.

Kati Basti: Localized Warm Oil Therapy

Kati Basti is one of the methods used for therapeutic purposes. Kati Basti is the method that consists of applying whatever warm medicated oil is appropriate to the back area for a period agreed on beforehand. This procedure is traditionally practiced with regard for lower back pain, stiffness, and other concerns associated with Vata. There have been studies on Kati Basti with respect to chronic back pain.

Abhyanga: Ayurvedic Oil Massage

Abhyanga (therapeutic oil massage) is a mixture of selection of medicated oils and special techniques of massage. Abhyanga may help relieve muscle tension and provide comfort and mobility to the patient. Based on the results of investigation of the use of Sahacharadi Taila massage (typically sesame oil) in patients suffering from chronic non-specific back pain or sciatica, the use of this method gives positive short-term results.

Patra Pinda: Leaf-Bolus Sudation, Leaf-Bundle Fomentation

Patra Pinda Sweda is referred to as an Ayurvedic treatment that is beneficial in the treatment of back aches and stiffness in which the practitioner uses warm herbs tied up in boluses made of herbal powders and medicines that are gently rubbed on the area. The heat and herbal properties of the treatment helps in easing tension in the muscles, enhance blood flow in the area of application, reduce 

stiffness and pain and aid in improving mobility. The procedure is generally regarded as a complementary form of treatment for mechanical back pain and muscle spasms.

Swedana: Therapeutic Sweating / Fomentation

Basti: Medicated Enema Therapy

Enema Procedures Basti is one of the Panchakarma therapies that are historically used for treating diseases attributed to Vata dosha. Depending on the patient’s condition, the doctor may choose other methods of administering Basti. It can only be performed in the medical establishment.

Panchakarma-Based Management

The following Panchakarma procedures may be used depending on the individual case:

Snehana → Swedana → Basti and/or other methods chosen by the doctor → Rasayana or rejuvenative procedures.

The order of medical procedures is based on the results of the examination.

Nevertheless, further studies are needed to prove its effectiveness.

Ayurvedic studies have researched options such as Snehan, Basti, Kati Basti and Agnikarma for treating Gridhrasi (sciatica). The results of the studies are promising, but the treatment must be based on the source of nerve irritation and neurological features.

Ayurvedic methods may be useful in managing symptoms in certain patients with disc problems. Nevertheless, cases involving serious neurological symptoms and important nerve compression must be evaluated by specialists and would not be suitable for Ayurveda alone.
